About the Role
We are currently seeking a compassionate, motivated, and experienced Community Support Worker to join our team. The successful candidate will be responsible for working directly with clients to assess needs, deliver tailored support services, and connect individuals to appropriate community resources.
Key Responsibilities
As a Community Support Worker, you will:
• Assess clients' needs and plan, develop, and implement tailored educational, training, and support programs. < r>• onduct interviews to determine the nature and extent of difficulties faced by clients, including emotional, financial, and social issues. < r>• M nitor and report on the progress of clients and adapt support plans as necessary. < r>• R fer clients to appropriate external agencies and community resources for further assistance. < r>• A sess community needs and available resources in areas such as health, welfare, housing, employment, and training. < r>• L aise and collaborate with community groups, welfare organisations, government agencies, and private businesses to address community concerns and improve service awareness. < r>• P ovide family support, education, and care to children and people with disabilities in various settings including group housing and care institutions. < r>• S pervise offenders on probation or parole where applicable and provide reintegration support. < r>• A sist young people in resolving personal, financial, and emotional issues, and guide them towards independence. < r>• P epare reports and submissions for funding, government reporting, and stakeholder updates. < r>
Qualifications:
• A certificate of qualification in Community Services, Social Work, Youth Work, or related field (preferred but not required). < r>
Other Requirements:
• P oven experience in client assessment, case management, and community engagement. < r>• E cellent interpersonal and communication skills with the ability to build trust and rapport with diverse groups. < r>• S rong knowledge of community services, welfare systems, and support programs. < r>• A ility to work independently and as part of a multidisciplinary team. < r>• D monstrated understanding of confidentiality, duty of care, and professional ethics.

What I Will Be Doing:
As a Support Worker at Northcott, I will empower customers to live as independently and confidently as possible — supporting them to build daily living skills, maintain their health and safety, and stay connected to their community.
- I will support customers with personal care in a way that promotes dignity and independence. This could include showering, toileting, continence support.
- I will assist with safe manual handling using equipment such as hoists and transfer aids.
- I will follow individual care plans to support health needs, such as epilepsy and wound care.
- I will administer medication safely and accurately according to medication management plans.
- I will prepare and support mealtimes according to mealtime management plans.
- I will help customers maintain a clean and safe home environment, for example light cleaning, laundry, shopping.
