An exciting opportunity exists for a Practice Improvement Specialist to travel across the North Coast region, from Port Macquarie to Goonellabah, driving quality improvement initiatives in aged care services.
About the opportunity:
An exciting opportunity exists for a Practice Improvement Specialist to use their skills and experience to implement, coach and guide the adoption of Uniting’s Senior Services models of care to improve service delivery outcomes.
Reporting directly to the Operational Support Lead, the role is responsible for embedding the service model principles across Senior Services streams, enabling the adopting of person-centred practices, promoting efficiency, and promoting opportunities for integration for service delivery.
Required to travel throughout the North Coast Region (From Port Macquarie to Goonellabah)
K ey responsibilities include:
•Coach and guide the adoption of Uniting’s Senior Services models of care and service provision to improve service delivery outcomes, with particular focus on Household Living across multiple Residential Aged Care services. This will include elements of training and delivering related education to influence changes in workflows, practices and routines.
•Oversee local practice improvement initiatives in close collaboration with the local operational support team and service delivery teams to embed sustainability of practice change to improve customer outcomes.
•Support and reinforce effective and efficient workflows and practices which reflect service model principles for Household Living.
•Lead and role model effective program and process reviews to support the adoption of models of care that align to Senior Services Operating Model principles and person-centred practice.
•Support the implementation of a consistent approach to underpinning ‘Continuum of Care’ and ‘Household Living’ model of service provision.
•Drive capability development and capacity building across services in the adoption of person-centred care principles/ models that promote positive outcomes.
•Obtain and utilise feedback from service teams to inform integration and improvements in service design, specific practice requirements and the Individual’s experience.
•Enable improvement initiatives to enhance the Individuals and staff experience across dimensions of quality, cost and service.
•Consider and recommend initiatives that support Uniting strategies and the Household Living principles, approach and business outcomes.
•Partner with Seniors Services shared services and key stakeholders to promote high quality and safe services.
Qualifications a d Experience:
•Certificate 4 in a relevant field or equivalent experience.
•This role will require experience in your field of expertise.
