This is a unique opportunity to play a vital role in supporting Aboriginal and Torres Strait Islander Elders to access culturally safe aged care services, while working within a supportive and community-focused team.
About the Role:
As Elder Care Support Coordinator , you will assist clients and their families to navigate the aged care system and access appropriate services that respect cultural identity, family, and community connections.
You will provide advocacy and guidance throughout the My Aged Care assessment process, support Elders and families to understand service entitlements, contractual arrangements and costs, and ensure care planning is culturally safe, trauma-informed, and meets client needs.
Working closely with the Elder Care Support Connector, you will actively engage with clients, families, and service providers to deliver holistic support and advocacy.
Job Details
- Contract : 12 months full-time, with the possibility of extension
- Salary : Competitive, based on experience + superannuation + salary packaging options

- Hours : Monday to Friday, no on-call requirements

- Support Elders and families to navigate aged care pathways and entitlements
- Provide advocacy to ensure cultural and clinical needs are reflected in care plans
- Assist clients with My Aged Care registration and assessments
- Liaise with aged care assessors and providers to ensure quality outcomes
- Help families understand contractual arrangements, costs, and service options
- Connect clients with internal programs and external services (SEWB, community engagement, etc.)
- Deliver face-to-face outreach to promote aged care services in the community
- Provide culturally safe, trauma-informed support throughout the care journey
- Demonstrated experience working in health, aged care, or community support roles (desirable)
- Experience supporting older Aboriginal and Torres Strait Islander people
- Strong advocacy and interpersonal skills to liaise with clients, families, and service providers
- Understanding of cultural safety and trauma-informed practice
- Ability to build knowledge of local services and establish effective referral pathways
- Excellent communication skills and a collaborative team approach

About the role:
Based at the head office in Bundoora, this mobile role involves travelling to clients’ homes across Melbourne to provide podiatric care. You’ll play a vital part in promoting foot health and preventing complications for older adults, especially those living with chronic conditions.
- Conduct comprehensive foot assessments, examining skin, nails, and circulation.
- Diagnose and treat common podiatric issues such as ingrown toenails, calluses, bunions, plantar fasciitis, and diabetic foot complications.
- Provide wound care and preventative interventions for high-risk clients.
- Develop and implement individualised treatment plans in collaboration with the Allied Health Team.
- Educate clients and caregivers on foot care, footwear, and chronic condition management.
- Prescribe orthotics and provide physical therapy recommendations when necessary.
- Accurately document assessments, treatment plans, and progress notes.
- Ensure all care is delivered in compliance with relevant clinical and regulatory standards.

Requirements :
- Tertiary qualification in Podiatry and current AHPRA registration.
