As a HCSW you would provide support to members of the multi-disciplinary and multi-agency Community Teams in delivering high standards of holistic person centred care to patients and their families/carers. Working with nursing, physiotherapy and occupational therapy departments on a rotational basis, providing the best quality care whilst under direct and indirect supervision. It is expected the HCSW regularly works unsupervised both in clinic and community setting.

Working with patients/clients within their own community setting and at time within the community hospital in relation to identified care plans and goal centred activity, focussing on the key priority areas of maximising independence and supporting the individual to remain in their own home. Working to help support and facilitate prompt discharge from hospital and to actively work in a preventative way to reduce/prevent admission.

To work within defined parameters and scope of practice often unsupervised with delegated case/workload to fulfil agreed / assessed needs with a person centred focus this will involve a variety of tasks within a multidisciplinary framework such as wound care, basic vital signs, venepuncture, catheter care and provision of equipment.

A full current drivers licence and access to a car is essential.
