+ This position is responsible to deliver patient centred care within the WA Home Therapies setting in accordance with organisational core values/standards of practice and codes and guidelines set out by APHRA.
+ Provides support to the management team by participating in the clinical management, education and quality activities of staff and patients within the training units.
+ Works autonomously within the centre, within accepted practice standards and with guidance and support from the level 2 CN's and the WA Home Therapies Manager
**Key Responsibilities / authorities:**
+ Participates in the design, planning, delivery and evaluation of patient care within the home dialysis therapies clinical setting including at times in the community throughout WA on home visits. This will include training patients/carers to manage either peritoneal dialysis or haemodialysis.
+ Protects the rights of the patient including maintaining confidentiality by adhering to Privacy Principles.
+ Functions as the primary co-ordinator of care for a group of dialysis patients, collaborating with other members of the health care team in the tertiary setting and community health care system.
Provides education and support to the client, to facilitate the attainment,
+ maintenance and restoration of optimal health and well-being.
+ Provides education, support and guidance to more junior staff including enrolled nurses working within the facility and works as part of a team to deliver excellent care.
+ Participates in staff education, acting as a preceptor and role model as required.
+ Functions as a positive team member, promoting the values, mission and goals of the centre and company, constantly working towards their achievement
+ Ensures/promotes a safe, secure and healthy working environment as detailed in the Occupational Health and Safety _Site safety Plan_ .
+ Actively participates in the quality improvement process by reporting all incidents, hazards and assists with the identification of opportunities for improvement i.e. auditing.
+ Participates in professional development activities, attending both internal and external education programs, to ensure maintenance and updating of professional knowledge and skills for clinical competence.
+ Completes clinical and mandatory competencies annually
+ Performs defined management functions when requested by WA Home
+ Therapies manager or Clinical Coordinator.
