About the role
Seeking a Level 3 Social Worker, who will provide psychosocial assessments and complex discharge planning for two inpatient oncology wards with a large elderly patient population. Work closely with medical teams to deliver patient-centred care in a supportive, part-time role.
Key responsibilities
- Deliver psychosocial assessments for oncology inpatients
- Lead complex discharge planning with multidisciplinary teams
- Provide support to patients, families, and carers
Skills & experience
- Degree in Social Work
- Current or eligible AASW registration
- Proven experience in hospital inpatient settings
- Strong skills in discharge planning and assessments
- Fully vaccinated for COVID-19 (including booster)

This Social Worker position sits within a multidisciplinary team offering coordinated rehabilitation services and discharge planning for older people.
War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24-hour inpatient care, Monday to Friday outpatient departments, iREAP Day Rehabilitation program, several specialist community teams inclusive of Transitional Aged Care, Geriatric Flying Squad, Young Onset Dementia, and day centre program, 24-hour accommodation service for regional guests and the full range of hospital support services. It is set within beautiful tranquil grounds and is a well-supported team environment.

- Ability to work effectively both autonomously and within a multidisciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
- Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients across all levels and departments within WMH and external stakeholders
- Current NSW Drivers’ license Class C li>Degree or postgraduate qualification in Social Work and eligibility for registration with the Australian Association of Social Workers (AASW)
