The Case Manager supports young people experiencing or at risk of homelessness through the delivery of our accommodation program. This role provides holistic, end-to-end case management—from intake to exit—offering advocacy, advice, and support. The focus is on helping clients access safe, stable, and affordable housing while addressing the underlying barriers that contribute to their housing instability.
Duties and Responsibilities include but are not limited to
- Provide responsive, person-centred case management to support young people experiencing or at risk of homelessness.
- Develop, implement, and monitor case plans in collaboration with clients, focusing on realistic and needs-based goals.
- Coordinate referrals and advocate for clients to access appropriate services and supports.
- Support clients to build life skills, independence, and self-determination.
- Maintain accurate records and complete regular reporting in line with organisational and legislative requirements.
- Collaborate with internal teams and external agencies to ensure timely and effective support.
- Assist with delegating tasks, managing service transactions, and attending relevant training and development activities.
- Take a lead role in team meetings and contribute to continuous service improvement.
Essential Requirements:
- Diploma qualifications in community services, mental health, youth work or equivalent.
- Previous experience in case management or relevant work experience with young people.
- Current driver's licence
- NSW Working with Children Check (Service NSW)
- First Aid Certificate (or be willing to obtain)
What you need to succeed:
- Experience working with complex clients , including children and young people impacted by trauma, abuse, or neglect, with insight into the effects on behaviour and development.
- Familiarity with using outcomes measurement tools to track client progress and inform service delivery.
- Skilled in conducting assessments, making referrals, and connecting clients to appropriate support services.
- Solid understanding of child protection, as well as client vulnerabilities including substance use, mental health issues, and legal challenges.
- Knowledge of the complexities and impacts of domestic and family violence , including legal and social implications.
- Excellent time management and organisational skills , with the ability to prioritise and manage a varied caseload.
- Confident using office technology and proficient in Microsoft Office applications (Word, Excel, Outlook).
